We welcome and celebrate persons of all cultural backgrounds and faiths. Basic requirements for residency are as follows:
-You must be 55 years of age or older;
-Able to ambulate or transfer to and from wheelchair without assistance;
-Continent or able to manage own incontinence;
-Must not pose a danger to self or others; and
-Must be able and willing to cooperate in problem solving.
If you feel that Dayspring Villa is a good fit for you or your loved one, the first step to becoming a resident is filling out and returning the application included in the information packet, along with the following information:
-Physician's Plan of Care form (in packet) filled out by your physician;
-Copies of Medicaid, Medicare, insurance, and identification cards;
-Copies of most recent bank statements;
-Copy of PAR from your Medicaid caseworker (if applying for a Medicaid unit).
Once we have all of these items, we will contact you to schedule an assessment to determine if we will be able to best meet your needs. If the assessment indicates that Dayspring will be able to meet all of the resident's needs, we will move forward with picking an apartment and scheduling a move-in date.
